A critical element of measurement-based care is the integrated team approach with regular review of all cases in the program. By reviewing a care manager’s entire caseload every week, this population-based approach to care:
- Ensures that a patient’s progress is regularly reviewed
- Provides consultation to the Ob-Gyn provider about recommended medication and treatment initiation, adjustment, or augmentation
- Utilizes regularly collected data to monitor a patient’s status
The case review team includes the care manager, an Ob-Gyn consultant, and a psychiatrist. Each patient – whether new, ongoing, or in maintenance – is discussed each week. This intense focus on a patient’s progress or lack of progress provides immediate feedback to the care manager and Ob-Gyn provider about suggested changes in treatment.
